When families are overwhelmed and unsure where to turn, Community Care Connections helps guide them through difficult decisions. We connect seniors and disabled older adults with available care options, placement support, community resources, and essential services.

We work alongside families, hospitals, case managers, and community partners to help ensure vulnerable individuals receive the support they need.

We do not directly provide medical treatment or caregiving services. Our role is to guide, educate, and connect families with trusted care providers and community resources.
