Eligibility Requirements

Individuals aged 50 and above, as well as those under 65 who have a Medicare-recognized disability. All other applicants will be evaluated individually to determine how we can provide the best support.

We do not directly provide medical treatment or caregiving services. Our role is to guide, educate, and connect families with trusted care providers and community resources.
